Donor EXPRESS now allows you to conveniently complete your pre-donation health questions on a smartphone, tablet, or computer! Donor EXPRESS must be done the same day you donate. It saves you time during screening and is confidential.

PLEASE CLEAR YOUR PHONE’S CACHE BEFORE COMPLETING DONOR EXPRESS to ensure you’re using the most up-to-date version of Donor EXPRESS so that your questionnaire works for your donation.

Donor EXPRESS is just one part of the screening process. Our staff will review your responses with you and administer various health checks when you arrive. Same day completion of Donor EXPRESS does not ensure you will be able to donate. Photo ID is required to donate.
